    Important Physical Examination Information
    Athletic physicals are good for 365 days; however, if the date on the original physical is more than 90 days before the first day of practice for each season, the parent/guardian must complete and submit a "Health History Update" form.  If the athletic physical is due to expire during a current season, the physical is still considered "current" for the remainder of that season.  A new physical will be required before the start of the NEXT season.
      
     
    You may have your personal physician complete the physical paperwork when the student-athlete has their annual physical. Because the EMO group in Berkeley Heights are our school physicians, all Sports Physical paperwork must be taken to their office for approval. They will sign and stamp it in the appropriate spot and you may then submit it to the Athletic Office.
     
    If you do not have your own physician, and there are extenuating circumstances, please contact the Athletic Office.
